Hengitysvaikeuspotilaan hoidon osaaminen perustason ensihoidossa : tietotestin kehittäminen

Abstract

Opinnäytetyö on osa Metropolia Ammattikorkeakoulun ja Keski-Uudenmaan pelastuslaitoksen KUOSCE-hanketta. Hankkeen tarkoituksena on luoda perustason osaamisen kehittymisen malli ensihoidon koulutukseen ja Keski-Uudenmaan pelastuslaitoksen käyttöön. Hankkeen avulla on tarkoitus löytää oppimisen kannalta parhaat käytänteet sekä esittää ja julkistaa toimintasuositukset perustason osaamisen vahvistamiseksi ja kehittämiseksi.

Opinnäytetyön tarkoituksena on kuvata hengitysvaikeuspotilaan hoidon osaamista perustason ensihoidossa ja kehittää hengitysvaikeuspotilaan hoidon osaamista arvioiva luotettava ja käyttökelpoinen tietotesti.

Opinnäytetyössä on kuvattu kirjallisuuteen perustuen hengityselimistön anatomiaa, fysiologiaa ja patofysiologiaa sekä hengitysvaikeuden ja hengitysvaikeuspotilaan hoidon osaamisen määritelmä perustason ensihoidossa. Hengitysvaikeuspotilaan hoidon osaaminen on jaettu ensiarvioon, tarkennettuun tilan arvioon, potilaan hoitamiseen, hoidon vasteeseen ja kuljettamiseen. Lisäksi opinnäytetyössä on kuvattu hengitysvaikeuden syyt sekä niiden hoito.

Kirjallisuuteen perustuen on kehitetty hengitysvaikeuspotilaan hoidon osaamista arvioiva tietotesti perustason ensihoitoon. Tietotesti koostuu 30:stä oikein-väärin väittämästä. Lisäksi opinnäytetyössä on arvioitu tietotestin luotettavuutta ja käyttökelpoisuutta. Tietotestiä ei tämän opinnäytetyön puitteissa toteuteta. Ennen testin käyttöönottoa pilotointi tulee kuitenkin toteuttaa testin luotettavuuden ja käyttökelpoisuuden varmistamiseksi.

This project was a part of the KUOSCE Project, which is ran by Keski-Uusimaa Rescue Department, Southerrn Finland, and Helsinki Metropolia University of Applied Sciences, Southern Finland. The aim of that project is to formalise the development of the core competences in basic-level emergency care. Thus, the model may be used for training among basiclevel medical technicians and emergency care students. Moreover, the purpose of this project is to find the best practicalities for learning and to present and publish recommendations for strengthening and developing the basic-level emergency care in Finland.

The purpose of this Final Project was to describe the core competences of treating a patient suffering from respiratory distress in basic-level emergency care. In addition, this Final Project aimed to develop a valid and useful knowledge test that measures the core competences of treating a patient with respiratory distress among emergency medical technicians(EMTs).

As for methods, this Final Project described anatomy, physiology and patophysiology of respiratory organs according to the literature of emergency nursing and other medical publications. It included also the definitions of respiratory distress and the core competences of treating a patient with respiratory distress in basic-level emergency care. The core competences of the treatment were grouped into five themes: primary examination, secondary examination, treatment and its response and transportation. In this Final Project, it was also described the reasons for respiratory distress and its treatments.

The knowledge test that measures the core competences of treating a patient suffering from respiratory distress among emergency medical technicians was developed based on the medical publications. The knowledge test consists of 30 true/false statements. It was also estimated the validity and usability of the knowledge test in this Final Project. The knowledge test has not been piloted yet. The piloting is going to be conducted by the students, who want to continue the work with the project.
