Tiivistelmä
Opinnäytetyön tarkoituksena oli kartoittaa leikkausosastolla kuolevan potilaan hoitotyötä narratiiviseen kirjallisuuskatsaukseen perustuen (n=18) ja luoda tarkistuslista kuoleman jälkeisiin toimenpiteisiin leikkausosastolla. Opinnäytetyön tavoitteena on edistää sekä omaisten informointia potilaan kuoleman jälkeen että yhtenäistää hoitohenkilökunnan työtä vainajan hoitoprosessissa. Toimeksiantajana toimii Turun ammattikorkeakoulu.
Maailmalla tehdään yli 230 miljoonaa kirurgista toimenpidettä vuosittain. Leikkauksen jälkeinen kuolleisuus on yksi tärkeimmistä tuloksista, jota voidaan mitata kirurgian erikoisosaamisalalla. Sitä käytetään yhä useammin mittaamaan hoidon laatua. Suomessa on tutkimusten mukaan matala leikkauksen jälkeisen kuolleisuuden riski. Leikkauskuolleisuuden on todettu lisääntyvän potilaiden ikääntyessä, mutta siihen vaikuttavat myös potilaan yleinen terveydentila, kirurgisen toimenpiteen erityispiirteet ja erityisesti se, toteutuuko leikkaus päivystystoimenpiteenä.
Kuolevan potilaan hoitotyötä voidaan pitää yhtenä merkittävänä osa-alueena terveydenhuollossa. Vainajan laitto kulttuuriset ja uskonnolliset eroavaisuudet huomioiden on tärkeä osa sairaanhoitajan ammatillista osaamista. Opinnäytetyössä analysoidaan, kuinka omaisille kuolemasta tiedottamista voitaisiin edistää ja analyysin perusteella kehitettiin havainnollistava kuvio. Opinnäytetyössä kehitettiin löydettyyn tietoon perustuen tarkistuslista kuoleman jälkeisiin toimenpiteisiin leikkausosastolla. Tarkistuslista on jaettu sisällöllisesti viiteen eri osa-alueeseen; vainajaan liittyvät tiedot, omaisten informointi, vainajan laitto, vainajan siirto vuodeosastolle sekä vainajan omaisuuden siirto vuodeosastolle. Tarkistuslistan käyttötarkoituksena on olla helppokäyttöinen apuväline kuolevan potilaan hoitotyössä. Tarkistuslistan jatkokehitystä ajatellen tulisi sen käyttöä tulevaisuudessa testata realistisissa skenaarioissa työelämässä. The purpose of this bachelor’s thesis was to find out about the nursing work of a patient dying in the surgical ward based on a narrative literature review (n=18) and create a checklist for postmortem procedures in the operating department. The aim of this study is to promote both informing relatives after the patient's death and unify the care of a deceased person in the operating department. The principal of this thesis is Turku University of Applied Sciences.
More than 230 million surgical procedures are performed worldwide each year. Postoperative mortality is one of the most important results that can be measured in field of surgical expertise. It is increasingly used to measure the quality of care. According to studies, Finland has a low risk of postoperative mortality. It is stated, that surgical mortality increases as patients age, but it's also affected by patient's general health, characteristics of the surgical procedure and in particular, whether surgery is performed as an emergency procedure.
Nursing care of a dying patient can be considered as one of the significant sectors of healthcare. Care of the deceased person and taking into account cultural and religious differences is an important part of a nurse’s professional competence. In the thesis it is analyzed how informing relatives about death could be promoted. Based on the analysis an illustrative figure was developed. A checklist was made for post-mortem procedures in the operating department. The checklist was based on the collected information. The checklist has been contentually divided into five different sectors; information related to the deceased, informing relatives, care of the deceased, transfer of the deceased to the ward and the transfer of the dead person's property to the ward. The purpose of the checklist is to be an easy-to-use tool for the care of a dying patient. For future development, the checklist should be tested in realistic working life scenarios.
