Tiivistelmä
Lasten sairaanhoito Suomessa on perhekeskeistä ja perhehuonemalliset lastenosastot antavat vanhemmille mahdollisuuden olla lapsensa vierellä ympäri vuorokauden. Perhehuonemallinen hoito tarvitsee osastoille vanhemmuutta tukevan hoitokulttuurin. Perhelähtöisessä hoidossa vanhemmat osallistuvat hoidon suunnitteluun ja päätöksentekoon, sekä huolehtivat lapsen päivittäisestä hoidosta yhdessä ammattilaisten kanssa.
Opinnäytetyön tarkoituksena oli saada tietoa vanhempien ja henkilökunnan kokemuksista perhekeskeisten hoitokäytäntöjen toteutumisesta lastenosastoilla. Tavoitteena oli saadun tiedon perusteella kehittää hoitotyön laatua perhelähtöisemmäksi.
Opinnäytetyö toteutettiin triangulaatio menetelmällä, jossa käytettiin sekä määrällistä, että laadullista tutkimusmenetelmää. Aineistot kerättiin haastattelemalla vastasyntyneiden osaston sekä lastenosaston vanhempia ja henkilökuntaa talvella 2019. Menetelmänä käytettiin perhekeskeisten hoitokäytäntöjen toteutumista arvioivaa Bliss Baby Charter Audit Tool itsearviointikaavaketta. Aineistot analysoitiin sisällönanalyysillä.
Tulosten mukaan osastojen perhekeskeisyys oli hyvällä tasolla. Vanhemmille tärkeintä oli saada olla lapsensa lähellä ympäri vuorokauden ja osallistua hänen päivittäiseen hoitoonsa henkilökunnan tuella. Vanhempien ja henkilökunnan kokemuksen mukaan lisätyötä tarvitaan edelleen perheen osastolle perehdyttämisen, lapsen yksilöllisen rytmin ja perheen yhtenäisen ohjaamisen alueilla. Tämän tutkimuksen tulosten perusteella vastasyntyneiden osaston omahoitajamallia ja lastenosaston kirurgisen lapsipotilaan hoitoprosessia tulisi tarkastella ja kehittää perhelähtöisemmäksi. Vanhempien ja henkilökunnan kokemukset osastojen perhekeskeisyydestä olivat tulosten mukaan hyvin yhtenäisiä.
Tuloksia on mahdollista hyödyntää lastenosastojen perhelähtöisen hoitokulttuurin kehittämisessä. Children's nursing in Finland is family-centered care (FCC) and nursing is carried out in single-family rooms at pediatric wards. This model gives parents the opportunity to be with their children around-the-clock. Accordingly, nursing in single-family rooms needs a care culture that supports parenting in the wards. In FCC parents participate in the planning and decision-making of care, as well as taking care of the child's daily needs together with professionals.
The purpose of the thesis was to obtain information about the experiences of parents and staff about the implementation of FCC practices in the NICU and pediatric ward. The aim was to develop the quality of nursing work to be more family-oriented.
The thesis was carried out using the triangulation method which used both quantitative and qualitative research methods. The data was collected by interviewing the parents and the staff of the NICU and the pediatric ward in the winter of 2019. The method selected the Bliss Baby Charter Audit Tool self-assessment form which evaluates the implementation of FCC practices. The data was analyzed with content analysis.
According to the results, the family focus of the departments was at a good level. The most important issue for the parents was to be close to their child around-the-clock and to participate in his/her daily care with the support of the staff. According to the experience of the responders, further work is still needed in the areas of familiarization with the family ward, the child's individual rhythm and the unified family guidance. Based on the results, the NICU primary nursing model and the pediatric ward surgical pediatric care process should be reviewed and developed to be more family-oriented. In addition, the results showed that the views of the responders on the wards' family-centeredness were very consistent.
The results can be utilized in the development of a FCC culture in NICU and pediatric wards.
