Tiivistelmä
Tämä opinnäytetyö käsittelee asiakkaan itsemääräämisoikeuden toteutumista kotihoidossa asiakkaiden ja hoitajien kokemana. Itsemääräämisoikeus on jokaisella ihmisellä, riippumatta iästä, sukupuolesta tai kansallisuudesta. Myös muistisairaalla ja kehitysvammaisella henkilöllä on itsemääräämisoikeus huolimatta sairauden tai vammaan tuomasta toimintakyvyn vajeesta tai heikkoudesta. Itsemääräämisoikeus on ihmisoikeus. Tutkimuksessa tavoitteena oli saada tietoa siitä, vastaako palvelutarpeenarviointi asiakkaiden tarpeisiin ja miten asiakkaiden itsemääräämisoikeus toteutuu kotihoidon eri osa-alueilla. Tutkimuksen kysymykset oli jaettu kolmeen osioon: palvelutarpeen arviointi, kotihoito ja itsemääräämisoikeus. Palvelutarpeen arviointi on merkittävässä osassa kotihoidon asiakkaiden itsemääräämisoikeutta, sillä kotihoidon asiakkuus alkaa palvelutarpeen arvioinnilla.
Tutkimus suoritettiin keskisuuren kaupungin yhden alueen vanhustyön kotihoidossa. Kysely suoritettiin paperilomakkein asiakkaille ja hoitajille. Kyselyn aikana asiakkaita kotihoidolla oli noin 730, kriteerit täyttäneistä asiakkaista vastanneita oli 100. Hoitaja vakansseja oli 124, kyselyyn vastanneita hoitajia oli 73. Asiakkaat vastasivat oman kokemuksensa mukaan, hoitajat vastaavasti vastasivat kuten olettivat asiakkaiden vastaavan. Tällä kyselyllä pyrittiin selvittämään vastaako hoitajien arvio asiakkaiden kokemukseen itsemääräämisoikeuden toteutumisesta. Kotihoidon tiimit valitsivat itse ne asiakkaat, jotka täyttivät annetut kriteerit ja joille kyselyä voitiin tarjota. Asiakkaiden vastausten perusteella voidaan päätellä asiakkaiden itsemääräämisoikeuden toteutuvan pääpiirteittäin hyvin. Palvelutarpeen arviointi on tulosten pohjalta onnistunut ja vastaa asiakkaiden tarpeisiin hyvin. Tämän opinnäytetyön pohjalta kehitysehdotuksena on, että asiakkaiden luona käyvien hoitajien tulisi vielä entistä enemmän kiinnittää huomiota asiakkaan toiveisiin, tahtoon ja oman elämänsä asiantuntijuuteen. This thesis deals with the realization of the client's right to self-determination in home care as experienced by clients and caregivers. Everyone has the right to self-determination, regardless of age, gender or nationality. A person with a memory disorder and a developmental disability also has the right to self-determination, despite the lack or weakness of functioning brought about by the illness or disability. The right to self-determination is a human right. The aim of the study was to obtain information on whether the assessment of service needs meets the needs of clients and how the clients' right to self-determination is realized in different areas of home care. The research questions were divided into three sections: service need assessment, home care, and autonomy. The assessment of the need for services is a significant part of the autonomy of home care clients, as the customer care of home care begins with the assessment of the need for services.
The study was conducted in home care for the elderly in one area of a medium-sized city. The survey was conducted on paper forms for clients and caregivers. During the survey, there were about 730 clients in home care, 100 out of the clients who met the criteria. There were 124 nurse vacancies, and 73 nurses who responded to the survey. The aim of this survey was to find out whether the caregivers 'assessment corresponds to the customers' experience of exercising their right to self-determination. The home care teams themselves selected the clients who met the given criteria and to whom the survey could be offered. On the basis of the customers 'answers, it can be concluded that the customers' right to self-determination is generally well implemented. Based on the results, the assessment of the need for services has been successful and meets the needs of customers well. On the basis of this thesis, the development proposal is that caregivers who visit clients should pay even more attention to the client's wishes, will and the expertise of their own lives.
