Tiivistelmä
Opinnäytetyön tarkoituksena oli kehittää terveytensä kannalta liian vähän liikkuville yli 75-vuotiaille ylöjärveläisille ennaltaehkäisevän liikunnan palvelupolku, jonka tavoitteena oli lisätä kotona asuvien ja toimintakyvyltään heikentyneiden liikuntaa ja fyysistä aktiivisuutta. Opinnäytetyön toimeksiantaja oli Ylöjärven kaupungin liikuntapalvelut. Kehittämistarve pohjautui Ylöjärven kaupunkistrategiaan, jota toteutettiin Ikiliikkuja-hankkeen sekä Voimaa vanhuuteen terveysliikuntaohjelman avulla. Ennaltaehkäisevät palvelut toimintakyvyn ja liikunnan ylläpitämiseksi ovat sekä kansanterveydellisesti että taloudellisesti tärkeitä. Ikääntyneiden fyysisestä aktiivisuudesta ja liikunnasta on annettu yksityiskohtaisia liikuntasuosituksia, joiden lähtökohtana on liikkumattomuuden välttäminen.
Opinnäytetyön toteuttamiseksi perustettiin poikkisektorinen projektiorganisaatio, joka kartoitti kohderyhmän tarpeita ja selvitti niiden toteutusmahdollisuuksia. Avainasemassa oli projektijohtaminen muutosjohtamisen näkökulmasta sekä palvelumuotoilu yhteiskehittämisen menetelmin. Toiminta toteutettiin julkisen sektorin, järjestöjen ja ikäihmisten yhteistyönä.
Palveluiden kehittäminen tapahtui palvelumuotoilun keinoin, sillä se sopi asiakaslähtöiseen palveluiden kehittämiseen. Palvelumuotoilussa korostui yksilön näkökulma, asiakasymmärrys sekä yhteistoiminta. Palvelumuotoiluprosessin osana toteutettiin kyselytutkimus, jota jaettiin mahdollisimman laajasti luotujen verkostojen avulla. Kyselytutkimus tehtiin ohjaamaan palvelusuunnittelua sekä lisäämään asiakasymmärrystä. Sen avulla selvitettiin, kuinka liikuntapalvelut tulisi toteuttaa, jotta terveytensä kannalta liian vähän liikkuvat yli 75-vuotiaat motivoituisivat terveytensä kannalta riittävään ja säännölliseen liikuntaan. Kyselytutkimus tuotti määrällistä tietoa, jonka avulla saatiin yleistä näkökulmaa, sekä laadullista tietoa asiakasymmärryksen syventämisen tueksi.
Palveluiden kehittämisessä näkyivät yhteiskehittämisen periaatteet. Palveluita kehitettiin tutkimuksen ja muutostarpeen osoittamalla tavalla. Toimintaa toteutettiin reflektoiden ja toteutumista havainnoitiin ja arvioitiin. Tämän pohjalta toimivia palveluita laajennettiin. Kehittämismenetelminä olivat käytössä vertaisarviointi, asiakasprofiilit ja aivoriihi, joiden avulla palvelupolku luotiin.
Vastaajat jakaantuivat paljon liikkuviin ja hyvin vähän liikkuviin. Liikunnan vähyys heijastui usein kaatumiseen, mikä saattaa kertoa toimintakyvyn heikkenemisestä. Ikääntyneet joko toivoivat ohjattua liikuntaa tai olivat omatoimiliikkujia. Heitä motivoi parempi fyysinen kunto ja toimintakyky sekä terveys- ja hyvinvointi. Liikunnan esteiksi koettiin sairaus, yksinäisyys, avun tarve ja etäisyydet.
Tutkimuksen tuloksia hyödynnettiin ikääntyneiden palveluiden kehittämisessä ja heidän fyysisen aktiivisuutensa ja toimintakykynsä tukemisessa. Palvelupolussa kohderyhmä jaettiin kolmeen kategoriaan: fysioterapian kuntoutusasiakkaat, omatoimiliikkujat ja liikuntaan apua tarvitsevat ikääntyneet. Fysioterapian kuntoutusasiakkaille luotiin kuntoutuksen jatkoryhmä, joka on kuntoutumista tukeva palvelupolku. Omatoimiliikkujille, jotka asuvat kotona ja liikkuvat terveytensä kannalta liian vähän, tuotiin palveluita lähemmäs kotia. Heitä varten koulutettiin vertaisohjaajia, jotka vapaaehtoistoiminnan puitteissa järjestävät liikuntaryhmiä ympäri Ylöjärveä. Kotona asuville apua tarvitseville ikääntyneille, jotka eivät pysty itsenäisesti harrastamaan liikuntaa, luotiin etäohjauspalvelu ja koulutettiin ulkoiluystäviä. The purpose of the thesis was to develop a preventive exercise service path for people over 75 years of age who do not exercise enough for their health. The service path aims to increase the physical activity of those living at home and with impaired functional capacity. The commissioning party for the thesis was the Sports Services of the City of Ylöjärvi. The need for development was based on the Ylöjärvi city strategy, which was implemented with the help of the Ikiliikkuja project and the Voimaa vanhuuteen health exercise program. Preventive services to maintain functional capacity and physical activity are important in terms of public health and economy. Detailed physical activity recommendations have been issued for the physical activity of the elderly. The starting point of the recommendations is to avoid inactivity.
To implement the thesis, a cross-sectoral project organization was established to find out the needs of the target group and investigate their implementation possibilities. Project management from the perspective of change management and service design using co-creation methods played a key role. The activities were carried out in cooperation between the public sector, organizations, and the elderly.
The development of services was carried out by means of service design, as it was suitable for customer-oriented service development. Service design emphasized the individual's perspective, customer understanding and cooperation. A survey was conducted to guide service planning and to increase customer understanding. The survey was distributed as widely as possible through the created networks. It was used to find out how sports services should be implemented so that people over the age of 75 who do not exercise enough would be motivated to engage in regular physical activity that is sufficient for their health. The survey produced quantitative information that provided a general perspective and qualitative information to support deeper customer understanding.
The principles of co-creation were reflected in the development of services. The services were developed as demonstrated by research and the need for change. The activities were carried out through reflection and the implementation was observed and evaluated. Based on the evaluation, the scale of the services was expanded. Benchmarking, customer profiles, and brainstorming were used as development methods to create the service path.
Lack of physical activity was often reflected in injuries, which may indicate a decrease in functional capacity. Elderly people either wished for supervised exercise or were independent exercisers. They are motivated by better physical condition and functional capacity, as well as health and well-being. Illness, loneliness, need for help, and distances were considered obstacles to physical activity.
The results of the study were utilized in developing services for the elderly and supporting their physical activity and functional capacity. In the service path, the target group was divided into three categories: physiotherapy rehabilitation clients, independent exercisers, and elderly people unable to exercise independently. First, a rehabilitation continuation group was created for the rehabilitation clients. It is a service path that supports rehabilitation. Secondly, services were brought closer to the homes of independent exercisers who live at home and do not exercise enough for their health. Peer tutors were trained for them to serve as volunteers to organize exercise groups around Ylöjärvi. Finally, a remote exercise service was created and outdoor friends were trained for elderly people who live at home and cannot exercise independently without help.
